Homeless Assistance Program (HAP)

Purpose:

The Homeless Assistance Program (HAP) provides services to persons who are at risk of becoming homeless (near homeless) or who are homeless. Participants must demonstrate that with the prevention and intervention services provided by the HAP service components, they would be able to meet their basic needs in the immediate and near future.

Homeless Management Information System (HMIS)

Purpose:

 In 2004, the Department of Housing and Urban Development (HUD) requested a new, digitally competent way to track homeless data for all of their homeless funded services. Their plan involves HMIS (Homeless Management Information System) software, a tool that provides a computerized data collection system designed to capture consumer information over time on the characteristics and service needs of all individuals experiencing homelessness. Emergency Shelters, Transitional Housing Programs and Permanent Supportive Housing Programs are key sources for data collection.

All Counties receiving HUD funding through the Continuum of Care process are mandated to be HMIS participants. Lack of participation by a County or agency receiving Continuum funding will result in ineligibility for this funding.

York County has all mandated HUD-funded agencies participating, as well as some other community agencies, in an attempt to get a clearer picture of what homelessness looks like in York County.

York County utilizes the Department of Community and Economic Development's (DCED) PAHMIS software.

 Human Services Development Fund (HSDF)

Purpose:

The Human Services Development Fund (HSDF) provides a flexible source of funding that can be used for a wide variety of human services programs. HSDF funds assist both categorical county government departments (such as Children & Youth, and Mental Health/Mental Retardation), as well as community agencies. HSDF funds are used to expand existing services, coordinate services among programs, and for other specialized or generic services. HSDF offers counties flexibility and allows each county to tailor their programs and cost efficiencies to the needs of their county.

 Medical Assistance Transportation Program (MATP)

Purpose:

The Medical Assistance Transportation Program (MATP) is designed to meet the non-emergency medical transportation needs of public assistance recipients. The MATP program concept was designed to:

  • Control the increased costs of public assistance recipient transportation
  • Eliminate past service delivery inefficiencies
  • Provide counties the flexibility necessary to manage delivery of medical transportation services

 State Food Purchase Program (SFPP)

Purpose:

The intention of the State Food Purchase Program is to supplement the efforts of food pantries, soup kitchens, regional food banks, feeding programs, shelters for the homeless and similar organizations in their work to reduce hunger.

The Emergency Food Assistance Program (TEFAP)

Purpose:

Originally, The Emergency Food Assistance Program (TEFAP) was initiated to distribute surplus commodities in government storage. Cheese, butter and corn meal were among the first commodities distributed. Currently, TEFAP foods include, but are not limited to canned fruits, juices, beef and pork, instant milk, dry pasta and baking mix.

Whenever surplus foods are allocated by the United States Department of Agriculture to Pennsylvania, they are re-allocated to each County Coordinating Agency based on poverty and unemployment levels in their respective areas.
